a) They can be naturally that big on a frame that small (although she looks like she over-diets rather than being naturally thin)
and
b) You must be kidding, they're up around her collarbone- they look hideous! The only boob jobs subtle enough to pass as real that I've ever seen have been in Japanese porn. You clearly need to study this topic more closely :)
